Perhaps because NRT and KIX are a significant distance from their respective city centers (50-75 km) to which most premium passengers would be traveling, meaning a car service would be prohibitively expensive for EK to provide?

Alternatively you can take a helicopter transfer - which takes 20 minutes and is 2500USD. Car included for last leg of your journey from the helipad.
HND is quite close to central Tokyo - therefore, more reasonable for taxis and car services.

Unfortunately HND services arrive at a relatively inconvenient time (a holdover from when the government only wanted Domestic stuff at HND and NRT to be International, I think, but perhaps now there is some err, lobbying to restrict more convenient slot times...!) - 2245 scheduled at HND whereas it's a 1720 arrival at NRT.
NRT does have the relatively convenient Narita Express train to Tokyo station where your hotel can arrange a much more reasonable transfer (or taxi from there), or the limousine bus, which I found to be quite decent - not a nasty tour coach at all but a quite pleasant road trip. Nice views (if you like that sort of thing).

Which are the busiest flights? Any tips on tours? Looking at about 10 days...

Difficult to give you advice without knowing what you are in to, who you are going with, how much you want to spend, how far you want to travel etc.! Perhaps the Japan forum might have some ideas?

Unless you have contacts in Japan then I would recommend emailing your presumably luxury hotel concierge to have them make the (sushi) restaurant recommendations right now...! Tokyo has no shortage of excellent cuisine, at all levels. And if the shopping in Ginza with the wife gets too much, go alone to an izakaya and get hammered...you won't be the only guy drinking alone haha!

On a personal note, I would recommend Sushi Saito over the other recommended "top" sushi restaurants in Tokyo, mainly because of course the sushi is probably the finest I have ever had, but also because the chef is very friendly and a bit of a joker! Only 8 seats though. But at least he's moved form the car park stairwell he was in before...

My family and I just got back from a trip to Niseko (skiing) and Tokyo (city thing). We were lucky enough to be referred to local guide for our Tokyo sojourn from friends who were expats there for a while. The cost for a private customized tour was 25,000 JPY (approx 210 USD) lasting about 9 hours. We loved her. (I have not quite 7 year old twins).

The EK flight also used to land at 1:30AM, after almost every public transport option except taxis were done for the day.
EK has the rights to fly to HND only during overnight hours -- land between 2200 and 0659, and depart between 0000 and 0659. Interestingly enough, I believe the A380 can actually operate at HND during the overnight hours, whereas they wouldn't be able to do so during the daytime due to HND's operational efficiency.
