There is some evidence that this can and does happen. I'd keep on it and check to make sure you are still actually on the waitlist. Don't feel bad about calling or tweeting to keep checking. Maybe they'll actually fix the problem when enough people keep calling.

I've done it before at the transfer desk no problem. I've got a tight connection which sucks but ill know in two week. Honestly ill probably get an op up anyways. Diamond on B fare with maybe Y9 left

I believe the res agents must code the following in the PNR's "E-Ticket Remarks" section:
eg. SWU1234567891234AMSATL03MAR13/SWU9876543219876ATLAMS09MAR13
explained: SWU[Cert No.][Origin][Dest][Day][Month][Year]/Repeat As Necessary
Then the agent must document the PNR using CMS canned OSI remark "PSGR USING SM SYSTEMWIDE UPGRADE"
I guess that's a very jargon-y explanation, but that is the process afaik from a res perspective.
