I believe on a BA A321, the crew member at the back has to remember to do the map, but on the smaller buses, it's the crews at the front who do it.

This can lead to it being forgotten on the A321.

Are you sure? I'm pretty sure those are topographic features in the ocean - i.e. trenches and the like. At least that is what I have seen.

Actually there are both. The shipwrecks have the year of demise. There seems to be some randomness to it, different shipwrecks on the same service in different directions. I can't remember if it was BA's system, but in some places around Malaysia and the Philippines there seem an awful lot of shipwrecks.
