Actually, you're right -- it looks like the first name input field is actually missing from the form. Gotta find out if you can feed it in somehow...

I've tried a few things, but I can't get my original window to unlock the name boxes.

I tried getting the Promo again by signing out of my Skymiles account and trying to book the same trip. No dice, got the 30K offer. Then I signed back in and tried a similiar flight. Score, got the 70K offer again.
So it is repeatable, but you have to be logged into Skymiles. And when I log into Skymiles, the link is tailored for my account and won't work for others it seems.
I still haven't accepted this offer- does anybody know if I can get two AMEX cards (Delta and SGP) within a month if you have 790 credit scores but got another AMEX card 2.5 months ago? My wife will kill me if we have to settle for a crummy hotel on our vacation because I couldn't get approved for the SPG.

Sign in to your delta account first. Search any flight, you will likely get an offer of 30000 miles and $50 statement credits. Then follow this post.
Once you got the url, switch the offer code to "U7-1000-46".
Since the url has your name and your sky miles account number embedded, you are good to go!

I tried booking three different tickets (SFO-LAX, PHX-LAX, SEA-JFK) and got only the 30k offer all three times. Was logged into my Skymiles account. Seems like this is only a targeted offer. i don't have a lot of skymiles in my account(~3000) and don't fly delta a lot. The last I flew Delta was over a year ago. Don't know if any of this matter though.
I might try once again tomorrow.
I might try once again tomorrow.

Nice trick. Actually, I just tried that, but doesn't seem to be working for me. When I change the offer code from U7-1000-76 to U7-1000-46 and send the request, my information is not auto populated in the 70k application.
