Must be enrolled in Family Pooling. One entry per day through 18 Feb 2015.

I think it may be 1 entry total. I entered yesterday and then today and it rejected todays entry.

why the need for a separate web address? and requesting my TB number and birthdate?

reeks of phishing although i'm pretty sure it's not

I'm certain it it not; I've been in touch with the people at JetBlue running the promo and it is legit.

As for why a separate address, probably to keep the load off the regular servers. I know there are other ways to do it but this is easiest. Also helps in branding the family pooling part of things.
