The upgrade was offerd to me at check-in in LHR. In fact, they had a small placard at the check-in counter advertising E+ upgrades. It was worth the money ($85.00), and that particular flight put me over 25,000 for the year, so I haven't had to worry about upgrading since.

I saw the costs written down at a human chicken counter. The charge for E+ went from about $25 to $100 depending on the mileage.
