It can only be done online. They can only walk you through it. I'm having the same issue. They said all we can do is keep reporting the issue, because they’re not able to process the upgrade requests themselves.

I've posted this issue on the UA FB group and their X account. Hopefully they'll respond. Site is still giving the same error.

It’s interesting that LH seems to be working right now, so maybe it is on the NH end for once.

Are you able to submit the request and confirm the upgrade? The issue is that no matter what flight information you enter, it never proceeds to the waitlist or confirmation page — it just keeps showing the same error. The Star Alliance Upgrade Awards form also keeps showing the same error.

This happened to me. It did not interfere with PP application, and my upgrade did eventually clear (C -> F). It did, however, prevent me from checking in online, and I needed to get it resolved at the airport (once control shifted to them). Before they even looked up my reservation I told them I had cleared into F but still had a lingering unticketed segment in business. The airport agent was able to remove the old segment and issue BPs.

Miracles do happen! Looks like someone fixed the NH upgrade software at least for using plus points. After trying for weeks unsuccessfully, this morning it let me get on the upgrade list for our January trip from NRT to BKK. So, keep trying....

Looks like they fixed the problem after I called the 1K desk yesterday, but now this error shows up for me. I found another agent on ANA direct and she managed to remove the UA duplicate itinerary for me and to add my UA MP number and other ID info to the itinerary. I'll wait until tomorrow to request upgrade again. Hopefully the error below won't happen to me!

If it still happens, any way to fix this?

SUCCESS! Interesting that I had to including my UA Ticket # to make it work, otherwise it gives me the error there is another passenger on the same flight! This is despite me getting ANA resolve my duplicate UA booking.

I never received this particular error message but I think the only potential solution to all of these error messages is the same approach. Keep calling United and keep trying online.

This issue was previously reported. The solution that typically works is to include your ticket number in the form, although the form says it is not required. It is probably the only error message that can be fixed without relying entirely on luck :)

I suppose with that error it's always possible that you do actually share a name with someone else on the flight, and the ticket number is needed for disambiguation.

Thank you njwatson32 and RoomChaser ! Indeed adding the 016 ticket # did the trick.
Fingers crossed for NH 159 on 10/15 and NH 160 on 10/19 to clear!
I've had zero luck on day flights but 100% hit on their night flights. How about the others' experience?
