I did Elizabeth line from T5 direct to Stratford then the DLR direct to LCY, 1 change but it does involve a change of platforms at Stratford (which has a lot of platforms!), It took about 75 minutes from when the train left T5

Between Custom House + bus or Stratford + DLR, I'd just decide on which train is first @ LHR (subject to engineering works, strikes or other incidents, obviously).

Interesting. The H30 runs for free between Heathrow Central and T4/T5 between 0312 and 0505 but there doesn't seem to be any public information online about the alleged T4-T5 service during the rest of the day

"The free H30 bus runs every 20 minutes between Terminal 4 and Terminal 5 via the Cargo Area from early in the morning until late at night. The H30 bus also serves Hatton Cross, Waterside, Compass Centre and Central Bus Station early in the morning."

On a separate ticket you aren't connecting. You won't have any protection on the second flight if the first is late.
You may think you have 1 hr 55 to make it but you need to knock off the gate close time from that so you're closer to an hour and a half.
Personally I'd fly the next morning even though a hotel will cost (since your arriving at T4 there are 4 hotels connected to the terminal via covered including a Hilton, Crown Plaza, Holiday in and one other whose name escapes me)

If it would be a financial disaster to miss the second flight, but rebooking it now is free/cheap then that would seem sensible. If you want to take your chances because it's not that expensive to buy a new ticket then that might also be an option.

Flying back to US out of T3 on delta. Is is possible to walk over to T2 to use the plaza premium lounge without having to be rescreened or go through border control again?

in fact you’d be screened three rimes even on the airside route.
on initial entry into T3
on entry into T2
on reentry to T3
