My favorite is the airport in Lesotho. That is a great shot of the runway leading to a drop off the cliff!

I immediately thought of DCA when I read the thread title. Not so many physical challenges as a ton of restricted airspace for captains to avoid. Plus the one runway is relatively short.

I've only landed at the two U.S. airports mentioned, DCA and JFK. I did land on a grass "runway" in Ecuador once. I don't think it was supposed to be scary, but it was.

Years ago (late 60's) LDY was served by Ballykelly, a military field on the east side of Lough Foyle (LDY is now at Eglinton). Arrival from GLA in an Air Ulster DC-3 was accomplished by crabbing sideways in the crosswind; that seemed quite a feat to this neophyte traveler at the time - IIRC, there was nothing but water all around.

In the early 60's, my very first flight (on a UA 707) MKC-JFK, I seem to recall acceleration well before turning from the taxiway onto the active - something about bridges or cornfields making for a shorter than desired runway.

Of course, those who have been there will remember that both DAD and HUI had their own idiosyncracies.

OOPS: Shoulda been MKC-IDL ... Kennedy was still President then.

A friend of mine took me on a leisure flight and did a landing and takeoff at CVL.

I haven't been to Saba, but I have landed in St. Bart's and it's insane. Check out this picture for a little more perspective -- it's the only place in the world I've ever gone where people hang out, watching planes land to see if they will make it or roll off into the ocean!
