I had my passport made in person at NYC, I repeatedly asked for the
48 page passport (with a note on the application)
I get my passport 2 hours later, nice and slim - 24 pages.. eh!
I guess I'll have to wait in line again to get the pages added.

From the AIT website: * IMPORTANT NOTE: Since September 2005, AIT and US Consular offices abroad are no longer able to extend or amend current US passports. If an American citizen overseas requires an alteration to passport information or a longer validity period for their passport, they must reapply for a new US passport using form DS-5504. See the section below for instructions on passport re-applications.

However ading pages is not what is traditionally seen as "extending or amending passports". Extending in DoS speak normally means the validity, and amending normally applies to things like name changes, etc. In September 2005 changes like this were instituted by ALL Embassies and Consulates abroad, but they all still add pages as well.
