In The Netherlands, I've flown into and out of AMS, and have departed from both RTM and EIN, but never arrived in either.
I have not flown in and out of MST and GRX which currently are 4th and 5th (LEY being in limbo)
In Slovakia, I've flown into and out of both BTS and KSC, and have, for a change, arrived into both TAT and PZY, but never departed from either.
That covers all airports with current commercial operations. There's also ZLA, which is open but no longer has commercial operations, only GA, and SLD, which is currently closed since 2020 for a never-ending renovation. Currently slated to re-open in 2027 (originally planned for 2022), and there may or may not be commercial aviation coming back.
Globally, I've only been to 3 of the top 10 - DXB, LHR and IST.

Went to ATL for the first and only time in 2023!
So sadly real answers are 22 SLC, 31 MDW, and most of the medium list!
And I would argue most of these airports are not hubs or even focus cities. Are they simply using hub as short hand for airport???

Good question. In the US I am missing only MDW in the large airports, SMF, SJU, OAK, CMH, PBI, OGG, BDL, CHS, OMA, and BUF in the medium list.

They're using "hub" in the US government sense. The US uses the "large hub", "medium hub", "small hub" monikers to classify airports. I don't know why the government uses that term instead of "airport" but Wikipedia is just copying that.
