OH - CLE used to be an actual hub for Continental and I think they had some intra-state flights.
KY - Similarly, CVG was a Delta hub and they had a pretty extensive regional network via Comair.

No one serves VIS anymore, they opted to take a cash payment from the federal government in lieu of EAS service. They have a 6x daily shuttle service to FAT, which is probably sufficient.

Wilmington/ILG had service from Eastern (1960s - 70s) and United (~1983-84); not sure who if anybody may have preceded Frontier, but I seem to recall that their inaugural was quite a big thing because the state hadn’t had much if any service for many years

Allegheny served ILG the same era as Eastern. I remember the two check in areas. According to wikipedia AA and TWA were the first airlines to serve ILG which I find amazing.
