So far on LH the FA not announcing anything, but one of the pilots usually within a minute before either happens: "cabin crew prepare for takeoff/landing" (time to really fasten the belt...).
With EW/GW it's sometimes only a BING or a light flashing before takeoff.

The FAA terminology was changed from "Position and hold" to ICAO-standard "Line up and wait" on 30 SEP 2010. Use of the older phrase is no longer considered correct by the FAA.

"Position and hold" has NOT been acceptable ATC phraseology in the United States for several years now.
"Line Up and Wait" is now correct ATC phraseology in the USA as with most of the world.
And while I don't want to study the FAA ATC manual in detail, I also believe that the "Cleared to" part is both redundant and not allowed as part of a LUAW clearance.
