DFW - UberX pickups allowed, there is a $2 toll for International Parkway (which runs through the airport) that will be added to your fare.

Please update LAX
LAX allows for all Uber pickup now, with the exception of UberPOOL.
When requesting uberX, uberXL, or UberSELECT at the airport, you'll be asked to confirm your terminal number and airline. Be sure to meet your driver at the "Ride Service Pick Up" sign nearest to your terminal. Uber recommends contacting your driver using the app to confirm your pickup location.

I finally used Uber at MSP. Follow signs to taxi area and use prearranged rides entrance. Best to summon once there if unfamiliar with airport as Uber arrival is fast.
$6 surcharge which is favorable to taxi airport charge and flag drop fees. Light rail to downtown Minneapolis is going to be cheaper and likely faster within a few blocks of the line. Other destinations, Uber beats taxi.

Technically, UberX isn't allowed there either - only licensed car services are allowed to pick up at BOS. Occasionally, if there are a lot of licensed car service drivers at BOS, some will be made available under the UberX pricing. I don't know if that means that the driver accepts the lower rate to get a fare rather than sitting and waiting, if Uber covers the difference between UberX and UberBlack rates, or if it's a blend of the two.

UberX and Uber Black are options at GRU as well.
UberX worked fine for me from GRU, after a short wait. If you have a lot of luggage, or more than 2 people, you'd probably want an Uber Black, as it was a smaller car (the quality of cars varied tremendously in Sao Paulo.)
The return trip to the airport went less well-- the driver got lost and took the wrong exit despite the airport being clearly marked-- but all my other Uber rides went quite well.
