The Middle East's newest program, Oman Air Sindbad, allows pooling of 8 immediate family members; these include "your spouse, siblings, children, parents, stepparents, grandparents and grandchildren".

Virgin's latest membership guide (warning large PDF) indicates that Gold members can set up family transfers in two ways:
1. A supplementary Silver account for one member of the household with miles pooled with those of the Gold account holder (not sure if this allows you to link another Virgin Atlantic account that already exists).
2. A 'family' account which allows you to earn points as the Gold holder for the travel of up to 3 individuals between 2 and 17 years of age in your household.

Etihad recently launched their FFP, Etihad Guest.
It allows the pooling of miles earned by up to 8 family members.

Club Tiare (TN) allows pooling and household a la BA.
Its called Club Tiare Fetii which im a proud member of. (but now that you can accrue on AA, ill most likely kiss it goodbye)
