I think it's a percentage tax, so the more expensive the ticket, the more it is.

Any sample dates or fare range? I'm only seeing $5K+ fares for AA and 10K+ for MH in late June or mid-July

dirt cheap coach is offered by italian low cost carrier bluexpress. one way in either way (incl. taxes) from 148 (http://www.blu-express.com/) - unfortunately no *A miles

Good fare but I can only see routing options via IAH making it ~8cpEQM on the CO option. If the EWR option came up it would be just under 5cpEQM and closer to a great deal.

The AA routing via MIA doesn't look so bad miles wise.
