I never send in the missing stay request form, I always call MR and explain the situation and ask them to research it while I'm on the phone with them. That way they can ask me any questions they have about the stay, and if they need to, they can call the hotel for confirmation. That way, when I get off the phone, I usually have a confirmation for the missing stay, pro or con.
