Looking for feedback on itinerary

Hello,
I am spending 13 nights in Japan in October. We would like to see a mix of urban, country, and traditional. I've put together a rough itinerary but I would appreciate some feedback before booking rooms.
The main questions at this point are a. are there any side trips we could cut out? b. Are we spending our nights in the right locations? c. should we venture out to Hiroshima given the other cities we are visiting? Basically the concern is that Hiroshima is pretty far away, so we aren't sure if it would be unique enough that we should visit. If we SHOULD visit, what should we cut out.

Yes, a lot of first-time travelers have no idea just how close together those cities are. Kyoto and Osaka are both on the Shinkansen, and if you have a JR Pass, it's about 20 minutes. Nara is not on the Shinkansen, but it takes, as I recall, 30 minutes from Kyoto, but certainly less than an hour.

I would set aside more time for Tokyo because your first/last nights will probably be spent arriving/departing and checking in. Tokyo has some huge & interesting prefectures and visiting more than one per day can sometimes be exhausting.
