It's not a luxury tax, it's a departure tax. They just (cynically IMO) figure people flying in the pointy end can afford to pay more.

Can I book round trip with stopovers in London to avoid fees?

Eg SFO-Munich, spend 3 nights in Munich, then Munich>London

Coming Back London>Dublin, spend 3 nights there and then Boston and connect somewhere in US?

Using what kind of miles? Some would allow these stopovers, some won't.

No matter what miles you use, you will still pay fuel surcharges if flying on British Airways transatlantic flights. If you were to somehow get to Munich from San Francisco on another carrier, you would not pay fuel surcharges on the outbound.

The fees would be less flying from London to Dublin to Boston than London to Boston directly, generally. But it depends on what carriers you are using and what miles you are redeeming.

And you also must always pay the UK Air passenger duty for UK departures. Is not applicable to transits.

Flying BA on an award requires hefty YQ charges.

Departing the UK invokes Air Passenger Duty - discounted for Economy, full for higher. Connecting in the UK under 24 hours invokes no APD. (There's a very useful wikipost in this thread in the AA forum dealing with the UK APD.)

I'm not sure whose miles you're using. If it were AA, I'd suggest checking TN (Air Tahiti Nui) LAX-CDG and going to LON on Eurostar. And to check connecting to airberlin (no fees), Ibera (lower YQ), etc.

Without knowing whose miles you're using makes it quite difficult for people to advise you.
