Probably because they checked against issued cards not active cards when issuing the new Intermiles card. Perhaps a mistake or they are trying really hard to compete with HDFC. All in all it's crazy risky for them to send out renewal cards with PINs (most banks have stopped sending PINs anyways) for cards that were cancelled 3 years ago!
ICICI has recently become very (misguidedly) aggressive with their cards. A month or two ago they increased the credit limit on my card (haven't used it in the last 15-20 years at least) from about 80k to almost 20L - and I don't have any other relationship with the bank for them to do this! It's as random as things get!
