Entry into Czech Republic by train

Entry into Czech Republic by train

I'll be visiting Vienna in December and was wondering whether as a vaccinated US citizen I would be able to enter into Czech Republic to visit Prague as well. Does anyone have experience with this or can point me to the relevant rules?

I am currently in Austria (US citizen) and planning to visit Czechia from Vienna in 10 days. My understanding is all I need (as a vaccinated US citizen) is to fill out and submit the following locator form:

https://plf.uzis.cz/

There are clear instructions in English on how to fill out and submit. I havent yet done so as I still need to buy the train ticket in order to provide the train and seat number required by the locator form. I have seen some reports that no-one was asked for this form when entering Czechia by train, but still intend to do it. Will report on my experience afterward.
