I dont think there have been any reports of bags not getting the tags in the first place it is what happens the other end that is the problem.

I've never had this problem, in fact one of the very few benefits of being Plat, is having luggage tagged with FIRST, regardless of flying class. And in most cities, it actually works that my tagged bags come off the belt first, or very early on.

I can imagine that ULD unloaders at some stations, if they fail to unload F baggage first or subsequent J baggage in order (through either a mistake or sheer laziness), might pull priority tags off as it gives them an excuse as to why the baggage did not get put on carousel in order.
For some, pulling some tags off is easier than dealing with an irate pax or dealing with an upset 'lost and found' colleague who may bear the brunt of the irate pax.

Landed in LHR last night, once again my first tags remained on my bags, (I was flying J) but my bag didn't appear until about halfway through. At least a 100 bags came off before mine arrived. More times than not, I do not get priority luggage when arriving at LHR, BKK is slightly better but still not consistent.

For me, everywhere except Africa delivers my bags first onto the carousel.
In Addis Ababa, Lagos and Entebbe, they can come out at any time. In Entebbe, airport staff have an annoying habit of removing bags to avoid overcrowding the belt. One time last year at Entebbe, I became convinced my bags had not been loaded onto the plane. As I walked across the hall to register lost baggage, I saw my 2 bags all alone on the floor!
I was one of the last to leave the arrivals hall, even though I was first off the aircraft as an F passenger!

Yes, it's the same in LGW. Two belts, one for 'premium' (First, Business and Gold - independent of cabin). Always worked so far, I have experience in all three 'categories'.

My experience as Plat is that my priority F tags do seem to fall off every once in a while, although luckily that usually only happens after they are loaded into the right container. It has to do with the quality of the tag material, which allows them to easily rip. And at some stations it also has to do with the fact of how they attach the priority tag to the baggage tag (eg., in ZRH): if it is too close and tight to the bag handle itself, it is much more likely to rip off.
