I used the code a while ago to sign up... and If I remember right, I was still charged the fee of 60$. I simply called in and they removed the fee immediately, no questions asked.

I have a 6-week Hertz rental in Italy approaching soon and received the following offer from SM:
"As a Medallion you'll always earn 200 miles per rental per day"
If so, how does this work - is there a membership card that I should show at the pick-up counter ?
