All the places you named are very small and you could managed doing things on your own. If you want to book an activity then here are some things we did.
Key west is very small. You probably don't need to book an excursion unless you want to do a water activity. The Conch Train will give you a good tour of Key West and you can hop-on, hop-off at the various sites.
Grand Cayman. Again really small. If you want a land tour you can walk out the cruise terminal and negotiate for a tour with the vans that will be waiting. If you want a water activity book in advance.
Cozumel. we have done two activities here. The Playa Mia cooking class was a lot of fun. Not difficult since they do all the prep work and you mainly just put stuff together. There is an open bar if that is of interest. The beach here is very nice and is included in the cooking class.
We have also booked a private car tour on Cozumel though here. http://www.toursplaza.com/categorias...iring_a_driver Gerry was great and we highly recommend him.
Castaway Cay is the private island so have no expirience there.

Grand Cayman if you've never done the stingray thing that's not too bad. Definitely better to go with a local operator rather than the ships tours. I did Captain Marvin's myself several years ago and it was pretty decent.
For just spending time on a beach, rent a car and head out to Rum Point. Food and drinks are pretty expensive out there, but it's a decent beach that's more out of the way from a lot of places, keeps down how many people are there.

GC: 7-mile beach, the turtle farm, or take the bus to "hell". The stringrays are really cool too.

You should pop over to Cruise Critic. They have port specific boards, so 20 or 30 mins reading over there and you'll have a list for each port. ONce you narrow it down, google to find tour operators. Much cheaper than ship's tours.

Grand Cayman: second the stingrays with Captain Marvin
Cozumel: a couple options for ruins: A) San Gervasio (rent a car, drive to the other side of the island, plus you can stop off at Rasta's for a drink, or one of the beach breaks for a swim or B) take the ferry to Playa Del Carmen and visit Tulum
Castaway Cay: Disney's private island. They (Disney) offers some things but given that they are the only action on the island, you're limited to what they offer. Make this your beach, swim day.
