Eszpresszo , if you have not already applied for the Altitude Reserve card, it appears that you are too late: US Bank does not seem to be taking new applications for the card. See here:

Did you try this year? I tried and it only charged $5.95, and not the rest. It was working last year.

Same, did not work this year. I had to settle for purchasing from vanillagift but they had a promo for 0 fees instead of the $5.95. Got 3 cards for $1500 using Paypal and collected 7500 URs, which is still 7.5% if you value them at 1.5 cpm. Not as good as stacking minus fee, but not terrible either.
