I don't think the middle name is an issue. The first and last names are the key. My passport has my full middle name while my OnePass account does not have a middle name or middle initial. I've never had a problem with immigration in the US or any other country I've entered.

I just spent a good hour on the elite line trying to do a minor name change on a ticket I bought for a friend of mine so that it could show up under his OnePass account.
I had booked it as "Initial. ChristianName Lastname" when his account is actually listed as "ForeignSoundingFirstname Initial. Lastname"
The first agent was unhelpful (and surly) stating that because "the first name is 'radically' different from the middle name" no change could be made and the "only thing that (you) can do is to submit the boarding passes in writing after the flight". I am not one to play the "race card" but I swear that had I been requesting a switch from "M. John Fox" to "Michael J. Fox" there would have been no problem at all.
However, once again I must thank you FTers for teaching me what to do...
I hung up, dialed again and got a new agent who was completely understanding and polite and fixed it in 5 minutes
