We've got plenty of hookahs in Boston too, they can be found mainly in the combat zone. Usually you see more in the summah than in the wintah, but I've been told that most of the hookahs are "smokin'".

I'm in DC so I won't be able to make it, however, I highly recommend trying Jasmine or Rose flavored sheisha if they have it available. It's a light smoothe smoke. I haven't been to a hookah bar in a few years, but we used to go every now and again when I was younger. Apple tastes okay as well, but the Jasmine is by far the best!

The hookah bars I've been too are in the DC area. They were more of a bar like atmosphere with the difference being that there is no alcohol served (maybe because of religious reasons? not sure) and it is mostly arabic men (when we went to them, there were a few women, but none were arabic, all were white).
The setting is very close and personal. Small circular tables with maybe 3 seats around each one. The ones that are in my area (Alexandria, VA) have some couches and that sort of seating. No one really ate food there (although they did have it available), it was more of a fruit juices/desserts and smoking (cigarette smoking was allowed and accepted).
I imagine that you can smoke throughout the meal, but if it were me going (and I am a cigarette smoker), I would only do it before and after the meal, not during the meal. The places never really filled up til later in the evening, we usually wouldn't go til 10PM at the earliest and they would be pretty packed, so I imagine that most would go ahead and have already eaten when it comes time to smoke. I have driven by them at earlier hours, like 6-7PM hour and they didn't seem too busy.
