These places are consistently highly rated -
C Restaurant

The Sandbar

Blue Water Caf
http://www.bluewatercafe.net/
Salmon House on the Hill

Monk McQueens

Joe Fortes

As for Vancouver Island, you'll have to be more specific - It's 460 kilometres long

I agree with "C" restaurant. It's almost exclusively seafood. The rainbow trout is very special. It has a strong ecological focus with the seafood it offers. (Actually, I guess rainbow trout is not seafood, technically speaking, but consider it anyway.)
Joe Fortes' ahi tuna was good.

Another one for "C" Restaurant for just fresh seafood cuisine.
Tojo is the best Japanese Restaurant by far.
If salmon is what you want to eat, Salmon House on the Hill is great.
With the 911 security measures, the Cannery is now a pain to get to but the scenery is pretty good. So is Horizons Restaurant.
