KWA (Kwajalein)

Runway takes up a lot of the island which is only 3 miles long & half a mile wide (at its widest point). No fences, either.

Bora Bora - the only way to get to the airport is by boat.

Ketchikan, Alaska - requires a 7 minute ferry ride to access it

The wait time for the HKG > Macau hydrofoil can be infuriating. But it's only about a 60 mile swim. I hear that the South China Sea is lovely (and 'relatively' unpolluted and shark-free) at this time of year.

Gibraltar (GIB) could also be swum to, and you could avoid the border crossing if you come from the Spanish side

Google Maps used to extensively recommend swimming across the ocean. Nowadays, though, it seems to have done away with most of that. But, for example, directions from NRT to SEA airport do call for a 34-day kayak trip with a break in Honolulu to relax. (http://g.co/maps/4abza)
