On board comment forms? Do they exist?

On board comment forms? Do they exist?
I know sometimes on international J class bookings the head steward will near the end of the flight address you by your last name and inquire as to any comments you may have regarding the flight.
My question is, do they have physical paper based in flight comment forms that you can fill out on your pull out table tray while whatever positive/negative comments are still fresh in your head?
Reason I asked is that the head steward on my last J class flight to NRT out of YVR was extremely professional and courteous to all of us. Was quite refreshing. I would have liked to of been able to comment on his positve attitude by filling out one of those forms.

The comment forms are in the forms satchel and each FA should be able to get one for you to complete. It is more like a mini survey though, so writing Air Canada directly will be more likely to get on the right person's file. The head steward is called a Purser or Incharge.

The forms are ones for Customer Solutions. The ICs are usually happy to give you one, and will often fill out flt#, seat# etc. Eventually you get a form letter back from CS sayign they appreciate the comments and will tell the crew.
