Retrain doesn't just mean that you need to remind them of the rule. Retraining also is important to emphasize the criticality of the rule and importance of enforcement.
If this FA has a history of not following rules and not doing their job, then they should be terminated. Unfortunately, AA doesn't seem to ever do that.

On the mainline jets, I believe they do, for standardization and having an extra in case of equipment failure. Can't remember for sure - good point though, one more risk on top of the many others identified in the thread.
