Since they required the names on our profile match passports it has gotten ridiculous. I never use my formal first name -- and my brother uses his middle name and we both find the way we are addressed on the phone to be irritating. And it didn't used to bother me so much, but now that I am an Old Fart™ I would rather be addressed as "Mr Brewer" and let me say "call me X"
A couple of years ago we moved just far enough away that it was easier to set up with all new doctors. They all had that space for stating how I wanted to be addressed, so I put in Captain Awesome. It's been 10 years and my dentist and eye doctor still use that when they mail appt reminders and when I check in.
