Has something changed? I am about to have another RTW issued (16 segments which include a journey to NYC (into LGA, out of JFK). Are you saying that LGA-JFK will now be treated as an additional segment, thereby putting me over the 16 max?
Yes. The rule changed over two years with the addition of language noted above by christep. It has been discussed a number of times over the years including in these threads (among others):

(h) A minimum of 3 and maximum of 16 segments, including surface segments between
any 2 airports, are permitted for the entire journey. Free flight segments within each
continent are limited as follows:
