New Passport Question / Problem / Divorce

UPDATE:

I actually called the passport office at the State Department. The nice gentleman told me just put down what I know and attach the divorce decree - although it's really not needed. All of it is just a formality. I'm good to go on everything. When it's renewal time in a decade - it'll be a non-issue.
