When booking on ual.com, always log in as a guest and you will be provided with a place to provide your DM (or any other *A/partner) FF number.
Now that it's already booked, your best bet is to call UA reservations and have them attach your DM number to the reservation.
That doesn't always work, so make sure your number is on your BP when you get to the airport. If it isn't, insist they print out a new BP with it on. Don't let them tell you "oh, it's in the system". Trust me, it isn't.
