On about 90% of the time. I always ask and only once recently in ORD at night when I think the pilots were frazzled did they say no.

12/4 ORD-DEN On during takeoff but off during approach to DEN (wasn't listening the entire flight, so I don't know exactly when it got cut off)

Dont remember for each flight in the beginning of November, but if it's not on, they always turn it on when I ask (and I always do!)

PDX-ORD-GRB-ORD-PDX every two weeks for the past seven months: Channel 9 always on from gate to gate on supporting aircraft. I really miss it when on the CRJ segments to and from GRB.
My one PDX-SFO-LAX-ORD-GRB-SFO-PDX (had to catch that domestic 744!) run: On from gate to gate except for the 744 flight, where it was turned on just after pushback.

UA307 BWI-LAX it was turned off. I asked but the FA said "he never turns it on."
All of my other flights in recent memory (at least a year, maybe two) it has been on.
