question on imposition of fuel surcharges

I couldn't find the appropriate forum for this question so please don't be offended.

I was wondering if anyone would be kind as to direct me when and which airline imposed their yq or fuel surcharges and which website contained historical crude oil prices most accurately. Im particularly interested in american airlines, united airlines, qantas and cathay pacific.
