American
America West
British Airways
Braniff
Continental
Eastern
Hawaiian Air
Lufthansa
Olympic

More of an asia-pacific carrier mix for me:

Singapore Airlines
Royal Brunei
Malaysia Airlines
Garuda Indonesia
Cathay Pacific
Emirates
Qantas Airways
British Airways
Virgin Blue
Japan Airlines

As you can see from the list I've been pampered once too many by asian hospitality, so riding on Qantas and BA can be rather dreadful. My favourite from that list is still Royal Brunei though, lousy hub, old planes, crazy connection times, but I love it for its extremely affordable (and I mean cheap) J class seats (esp. now that they have the SQ Skybeds-type seats) and genuine crew whom don't give you the impression that the next thing they do is to go to the galley behind to ***** about ya (not that they don't, they just dont carry that impression imho). The new sky lounge in brunei is one of the most welcoming ones, and the buffet, while it doesn't quite serve gourmet stuff, will fill you (as compared to sandwiches and nuts in many many qantas clubs that I've been to).
