Northwest Airlines / Mesaba
Delta
USAir

Virgin Atlantic

I also once flew Madrid to Brussels in 1999, but I can't remember on what carrier. Perhaps Iberia?
I don't think I ever flew Continental, but it's possible. I flew a multitude of airlines from CHI to NYC/ISP when I was younger, so I don't know if I've remembered them all.
