EC 261/2004 experiences

There are many instances on various airlines of EC 261/2004 experiences. The purpose of this thread is to consolidate that information and provide anecdotal evidence of claims being accepted or denied. Please share your experience!

I would kindly ask that you include:
- some details about the situation (flight delayed / cancelled / downgrade...)
- the operating carrier
- the ticketing airline
- whether the claim was accepted or denied (or initially denied and later accepted)
- whether a third party organization (such as EUClaim) was involved

If you posted this info in another thread (one related to a specific carrier), it would be helpful to post only an "executive summary" and maybe a link to that thread.
