This. he will have no fear after taking xanax before and during air travel.

For flying the doctor will probably prescribe a large dose which would probably be perfect because he will be able to get some sleep on the flight as well. If he has physical symptoms from the fear of flying he can be prescribed beta blockers which do a good job of getting rid of those as well. Good luck!!

This situation fits the rules for an open jaw without going into an explanation of what they are.
It's not an open jaw because there is no one airline that flies SFO-BCN and VCE-SFO, or SFO-CDG and MUC-SFO, for that matter.

On open jaw would be, like rotanes suggests, on Delta SFO-ATL-BCN/VCE-ATL-SFO (I think that US Airways also serves both BCN and VCE), but, since there isn't one airline that provides both non-stops, it would be two one-ways. (You could certainly go to a site like Orbitz and type in the open jaw itinerary SFO-CDG/MUC-SFO, and it would return an itinerary, but for the non-stop option it would be priced as the sum of the two one-ways on different airlines.)

What a great advice being potted before the flight and then champagne during the flight... You better already call the ambulance to pickup the person after landing

It would be easier to address the fear of flying issue. Encourage that person to get a benzodiazepine perscrioption for the trip to reduce anxiety. This should make it easier to travel with them and to book more economical routing. Like it or not, you are going to have to take more than one plane.

Ask them to take a free course at fearofflyinghelp.com (fear of flying help) which is written by an American Airlines pilot.

Ask them to see a psychiatrist. Seeing a psychiatrist doesn't mean you are bonkers. Medication may or may not be the answer. It may be part of the solution. For example, medication + psychotherapy + fear of flying course + a particular flight routing (maybe a wider plane, maybe flying through Paris both ways so that there will be a sense of familiarity, etc).

Also consider the non-stop Los Angeles to Madrid and then try to fly from Venice to Madrid to Los Angeles. Another way is to ask the person to drive from San Francisco to New York and fly non-stop JFK to Barcelona and back from Venice.

Bon voyage!
