When my wife got the passport card through the passport renewal procedure, she got her passport book back with a photo stapled on the signature page. They also put a rather large scratch on the bio page 
She removed the photo, but two small holes from the staple are still visible. At the same time, the scratch on the bio page is not large enough to raise questions from border officials but is still very annoying. It seems like incompetence is the primary qualification to be employed by the federal government
