The answer is: it depends.
Depends on which airport, depends on which lounge, depends on which matron, depends on the mood of the selected matron at the time you're trying to enter the lounge, depends on how crowded the lounge is, depends on whether you have something that they can rely on to verify your right to enter, and so on, and so on, and so on.

It sure does depend. Even LH who are infamous for requiring card at their home airports especially, did not apply that consistently on my recent trip (with multiple transits and lounge visits at FRA and MUC).
