Erm ... let's be careful here. You say

but that 'first' should have an asterisk or something because it is not the case that dates are 'fully changeable without penalty' simply after the first chronological flight there are explicit conditions spelled out in the rules.

Also, you say

Has this charge gone up very recently? In the January '07 version of the rules it is only $125.

I don't think that's entirely true for travel originating in Europe. I think the rule still says that after departure, no changes are permitted "up to and including first intercontinental sector"
