Not necessarily: some airlines, typical European ones, will charge say Euro 50, even on fully refundable tickets.
Best to play such tricks with a travel agent (not online) you know.

In other words, they have to "approve" the validity of your CDW, or you have to pay an equivalent of it.
Amex Platinum isn't on their approved list apparently.
