Click on office/business in the category list, then click on business center/Office Depot. There's a one-question, 5-point poll on the right near the bottom of the page.

Stumbled into it. I spend a small fortune at Office Depot through mypoints and clicked to it through the category list. Was looking at the featured offers and happened to scroll down far enough.

I found it today. Just as described. Welcome page -> Quick Search (on right side of page): Office/Business -> Business Center ad (on right side of page) -> Poll on bottom right side of page.

I've noticed there are two OfficeDepot listings on MyPoints (Office/Business). One for purchases over $50 and one offering 3 points per $ spent if you're making a smaller purchase.
