Pretty sure Polaris is only for DEEP S. America, and LIM is excluded. Does get United Club Lounge and lie flat seats, just not Polaris lounge or onboard Polaris service.

Polaris applies to LIM (having flown this from IAH). You can access the lounge both on the outbound and inbound and it is Polaris service on a 767.

The fare is still alive. In fact, I just bought a second ticket, and might be able to visit both Macchu Pichu and Lake Louise in one ticket for $1,222, without taking any PTOs!
