Just called Amex, we talked about the promotion and I directed him to the purchase I just made. The rep confirmed for me that the purchase was eligible for the $400 back, and that I should receive it within 90 days. I asked him to add a note to my account just to be safe.

While I'm not an expert on AS's Mileage Plan, it can if you route it right. Both QF and EK are partners, but under QF you have to be on QF metal.

Those EK metal flights are in the 8xxx range.
Take BA to LHR and take QF metal as far as you can.
To qualify for mileage accrual, please note that your flights must fall within the eligible flight number range below and your flight must be marketed and operated by Qantas.

No tickets issued for me either. I booked directly with Qantas about 18 hours ago. I did notice that when I went to the QF site today there was a schedule change notification on DXB-SYD (flight moved 5 mins later) so maybe it was just hung up there.
