"...snip...you can change dates, flight numbers and carriers at any time (for all but the first international sector) for no charge. ..."
I haven't been 100% successful in getting the airlines to agree to that statement. At least a few months ago AA LAX ATO insisted on USD75 to change a LAX-LHR segment from BA to AA. (I walked over to the BA counter, and they stickered it for free, but AA would have none of that.) If they can make up their own rules on matters that obvious, I don't hold out much hope for anything grander.

As your experience shows, changing carrier on a route, keeping origin and destination unchanged, does not require a ticket re-issue. This is clear as BA could do it by just stickering the original coupon with the flight details change.
Perhaps the AA agent did not realise that change does not require a re-issue. If that is the case, then they are wrong. Or perhaps AA has a different policy about stickering a flight coupon??? I have regularly made changes to date/time, but I usually don't change carrier so have not run into any interpretation problems.
And remember that the airlines can choose to charge their own service fee on top of the service charges outlined in the fare rules. This is often the case now with paper tickets vs e-ticket. Perhaps AA are applying their own service fee and were not actually trying to recoup via the fare rules??

At the same time, I was in fact asking AA to reroute a second unrelated OWE. At LAX "ATO" (looks like a normal check-in line to me, sorry for the pax behind me) the process was so long and painful that no money changed hands.
