Easy question?
I'm sure this probably qualifies as a "frequently-asked question" but I've not found an answer using Search...
Hypothetical example:
#1) Rate for city B to city C $400
#2) Rate for city A to C, with a connection in B $200
If you book the A-B-C option and don't fly the A-B segments, is there any hassle or penalty? In my case, I live by the City B airport and City A is a 2hr drive, so I'd rather skip that segment if possible (while still enjoying the lower rate).

This only works if your travel ends in city B.
By not using the B to C portion of your ticket, ALL subsequent flights; the connection from B to c and the return flights from C to B, then B to A will be canceled.

If you don't fly A-B, your whole itin is cancelled. You can fly A-B & drop C, but that's hidden city ticketing & that's against airline policies (but only if they catch you - and only if you do it regularly. I'm assuming since you're in B, you're in a hub city? If you can find a cheap one-way B-A flight (under the $200 difference), you could fly B-A-B-C & return C-B & drop B-A on the return - again, technically against airline rules (but again, only if they catch you).

That said, if not checking bags, and if you can be dropped off at A, fly from A but disembark in B. Such as:
A-B-C
C-B (get off the plane)
It's admirable to tell the gate agent that you won't be boarding the final B-A flight. Then they won't page you incessantly when the door is closing.

I haven't seen anyone mention it but if you drop the B-A segment (especially on a regular basis) you wouldn't want to use your Worldperks number.
